COVID-19 Vaccination Clinic returns to Sunnybank tomorrow

Metro Health South will run another COVID-19 Vaccination clinic at Autism Queensland, 437 Hellawell Road, Sunnybank Hills on Friday 13 and Saturday 14 May, 8.30am-4:00pm for clients, their family members and everyone else requiring their first, second or booster jab.
Drop-ins are welcome, however Queensland Health recommend booking an appointment by calling the special assistance phone line, 3176 7799 or emailing [email protected].
This will provide you with the opportunity to discuss concerns you may have and to share important information with clinicians about the person receiving the vaccination such as needle phobia, sensory issues etc.
You are also asked to download and bring a completed consent form with you.
Child (5-11 Years) Consent Form
Youth (12-17 Years) Consent Form
Adult (from 18 years) Consent Form.
